To complete this form, you must be an elected school-based teacher of the year at public or charter school in Jacksonville, Florida, and your principal must have sent your nomination form to the Jacksonville Public Education Fund by August 30, 2019. If you have any questions about your eligibility for this award, please contact us at coordinator@jaxpef.org.

The application is completed solely online. The Smarter Select software will require you to create an account so that you can log in and out to update your application over time. The software automatically saves your information as you work. However, we strongly encourage you to complete the essay questions using word processing software so that you can save a copy locally on your computer.

To complete the application you’ll need have the information for the following sections:

Section One - Personal Information
  • Your contact information
  • School demographics and information
  • FLDOE Certificate Information
  • Resume (MUST be a PDF* and it must be typed in 11-point font with a two-page maximum)
Section Two - Questions About Your Teaching Practice
  1. What inspires you as an educator? (250 word max)
  2. Describe the strongest evidence available to demonstrate your success at advancing student learning. Using data, formal/informal and short/long term, how are your students performing when they enter your classroom versus when they leave? (350 word max)
  3. A teacher leader is someone who affects student outcomes in classrooms beyond their own. Describe how you have been a teacher leader in your school, district or subject area. (500 word max)
  4. If selected as the Duval County Teacher of the Year, you would serve as a spokesperson and representative for the teaching profession. What would be your primary message? (250 word max)
Section Three – Letters of Recommendation

Three letters of recommendation. One MUST be from your current principal. The other two could be from any combination of the following: student, parent, community member, or colleague. Section Four – Acknowledgements

If you are selected as one of Duval County's top five finalists for District Teacher of the Year, you will need to select a colleague from your school as your designee to help with your participation during the EDDY Awards. You’ll need this designee’s name and contact information. In this section, you’ll also confirm your participation in the 2020 EDDY Awards Experience and the 2021 Florida Teacher of the Year.
